Moving between children’s and adults healthcare

How you will be involved in decisions about your care as you get older

You may have used or been involved with one or more of the following healthcare services

  • Integrated Therapy Service for Children and Young People (ITS)
  • Child and Adolescent Mental Health Services (CAMHS)
  • Paediatric Continence
  • Children Looked After

When you reach 14 years old, the service you are involved with will start to talk to you about a ‘transition’ care plan and what transition will mean for you. This will depend on a number of things, such as your current skills, abilities and needs, as well as the nature of your underlying health condition.

Some aspects of your care will not need to transition but some will. We will work with you to start talking with the adult services that may be involved in your care, so that when you reach 18 your transition of care will be easier and you will have been involved in each stage of the process.
